These documents are provided to assist CORE 42 IHEs in collecting the information needed for the portal. IHEs are strongly encouraged to use the example forms to collect the data needed for the portal and as an emergency backup if the portal fails. DO NOT email proposals or reports unless MDHEWD instructs you to do so.
Cycles are named for the academic year in which approved equivalencies will first be recognized, not the year in which proposals or reports are submitted; proposals and reports submitted during 2026 are reflected in the AY 2027-2028 database.- NEW MOTR Equivalent Courseproposal (to recommend a CORE 42 IHE’s course for first-time equivalence to a MOTR number)
- SUBSTANTIVE CHANGE to a MOTR Equivalent Course proposal (to submit a change to a CORE 42 IHE’s current MOTR Equivalent Course credits, description, and/or learning outcomes for verification of continued equivalence and/or change the equivalent MOTR number)
- CLERICAL UPDATE to a MOTR Equivalent Course report (to make minor clerical updates to a CORE 42 IHE’s current MOTR Equivalent Course)
- DEACTIVATION of a MOTR Equivalent Course report (to deactivate MOTR number equivalence for a CORE 42 IHE’s course)

- The corresponding form (below) must be submitted in addition to the institutional course submission form (above) and will be reviewed for equivalency by the Missouri Mathematics Advisory Committee (MMAC), formerly Math Pathways Taskforce, to ensure the minimum Expected Learning Outcomes are included:
